If You Need to Take Leave of Absence

If you become ill and miss more than five days of work, you may be eligible for short-term disability. If a family member is ill, you may be eligible to take leave under the Family Medical Leave Act (FMLA) to care for your loved one or for your children.

Visit the Matrix website or call 1-877-202-0055 for information or to apply for a leave. To learn more about how to apply, watch this short tutorial here.

Physical Health

Telemedicine

With social distancing such an important strategy for stopping the spread of the coronavirus, now is the time to take advantage of your medical plan’s telemedicine service. Avoid going into a doctor’s office, urgent care center, or hospital — receive care right from your home instead. With telemedicine, you have access to board-certified physicians 24/7, 365 days a year. Get fast, convenient diagnosis and treatment for many common medical conditions through video consult on your smartphone or computer.

You also have access to virtual visits with behavioral health specialists who can help you through feelings of anxiety, stress, or depression.

Telemedicine is provided through Horizon CareOnline if you’re enrolled in a Horizon Blue Cross Blue Shield (BCBS) medical plan. The following Horizon Care Online services are provided at no cost to you through December 31st:

  • Assessments/diagnosis for COVID-19 symptoms
  • Medical and behavioral health visits

If you are in a PPO plan, you pay a $5 copayment for medical and behavioral health visits. If you are in the CDHP, you pay no more than $59 for medical health visits, and what you pay for behavioral health visits varies based on the service/provider. Deductible and coinsurance apply if you are in the CDHP.

Visit the Horizon CareOnline website to get started or learn more. You can also reach Horizon CareOnline at 1-877-716-5657.

Horizon Care Navigator/Primary Nurse Health Management Program

Managing a chronic condition can be more difficult during these unprecedented times. Your Horizon Blue Cross Blue Shield (BCBS) medical plan offers access to the Horizon Care Navigator/Primary Nurse Program to help support managing chronic conditions, such as diabetes, asthma and heart disease. They can also provide assistance with severe and complex care needs, including cancer, high risk pregnancies/NICU care, transplants and infusion therapy. The program’s trained case managers are fully licensed health care professionals and registered nurses.

Call today to speak with a personal nurse coach at 1-888-621-5894 option 2.

Medical Coverage

Your medical plan is always available to support you and your covered family members with comprehensive coverage. If you need COVID-19 testing, that expense will be fully covered at no cost to you, as long as your physician provides a prescription. In addition, all of our medical plans include an out-of-pocket maximum to protect you from extensive costs if you need lengthy medical care.

The Food and Drug Administration (FDA) has granted Emergency Use Authorization for three COVID-19 vaccines. This continues to change as new vaccine developments are made, and as the vaccines become more widely available. DSM employees and their covered dependents enrolled in a Horizon Medical plan will pay no out of pocket costs for the COVID-19 vaccine or its administration. See the communication sent to Horizon participants announcing this. To learn more about the benefits of getting the vaccine, how it was developed and how it works, see the flyer from Horizon.

View a summary of your medical plan details or log in to the Horizon Blue Cross Blue Shield (BCBS) website for more information about your coverage and plan resources.

Due to the COVID-19 pandemic, DSM has adopted legislative updates allowing for some midyear enrollment changes to be made, otherwise not allowed. To learn more about what changes can be made under this temporary law, review the Frequently Asked Questions (FAQ) here and contact the DSM Benefits Center to request the update: 1-866-353-9740, Monday – Friday, 9:00 a.m. to 7:00 p.m. ET.

Prescription Drug Coverage

Take advantage of your Express Scripts or Express Scripts’ specialty pharmacy Accredo home delivery program to receive a three-month supply of your maintenance medication without needing to leave your home.

Visit the Express Scripts website or call Express Scripts at 1-866-851-0145 to learn more or get started.

Flexible Spending Accounts (FSAs)

Based on the guidance provided by the IRS, DSM has adopted the following FSA changes. These updates are applicable to the 2021 plan year only and any changes impacting future years or additional changes by the IRS will be communicated as they become available.

Health Care and Dependent Care FSA

  • You are allowed to carryover the total balance of your account, and you will not forfeit unused 2020 funds.
  • You can increase, decrease, or make a new election without a qualifying event. Any decrease may not exceed the total amount of claims already paid out, so be sure to check your detailed activity with WageWorks.
  • If you elect to stop participating in an FSA during the year, you can submit a reimbursement request for claims incurred after your stop contributing, up to the amount you had contributed reduced by the claims that have been reimbursed.

Dependent Care FSA ONLY

  • Certain funds can be used to pay eligible expenses for an eligible dependent who is not yet age 14 (an increase in age from 13) during the 2021 plan year – if your child turned age 13 in 2020 and for expenses incurred in 2021.
  • This only applies to reimbursements from unspent amounts carried over from 2020.
  • You still cannot use amounts contributed in 2021 to pay for expenses of a child over age 13, and you cannot use DCFSA funds for a child who turns age 14 in 2021.

To learn more read this letter. To make mid-year change to your FSA account elections, contact the DSM Benefits Center at 1-866- 353- 9740.

Making Changes to Your Health Coverage

If you experience a qualifying life event — such as if your spouse loses his or her employer benefits, or if you have a baby or get married — you can make changes to your benefits coverage within 30 days of the event. Keep in mind that the changes must be related to your event. For example, if your spouse loses benefits coverage and you’re not currently enrolled in a DSM medical plan, you can newly enroll yourself and any eligible dependents; if you are already enrolled in a DSM medical plan, you can add your spouse to your current plan, but you cannot change which plan you are currently enrolled in.

To change your benefits enrollment, visit the DSM Enrollment Website website or call 1-866-353-9740 from Monday through Friday 9:00 a.m. to 7:00 p.m. ET.
