If You Need to Take Leave of Absence

If you become ill and miss more than five days of work, you may be eligible for short-term disability. If a family member is ill, you may be eligible to take leave under the Family Medical Leave Act (FMLA) to care for your loved one or for your children.

Physical Health

Telemedicine

Since social distancing is such an important strategy for stopping the spread of the coronavirus, now is the time to take advantage of your medical plan’s telemedicine service. Avoid going into a doctor’s office, urgent care center, or hospital — receive care right from your home instead. With telemedicine, you have access to board-certified physicians 24/7, 365 days a year. Get fast, convenient diagnosis and treatment for many common medical conditions through video consult on your smartphone or computer.

You also have access to virtual visits with behavioral health specialists who can help you through feelings of anxiety, stress, or depression.

Telemedicine is provided through Horizon CareOnline if you’re enrolled in a Horizon Blue Cross Blue Shield (BCBS) medical plan.

If you are enrolled in the CDHP, you will pay $0 for medical and behavioral health telemedicine visits regardless of whether you have met your deductible.

Horizon Care Navigator/Primary Nurse Health Management Program

Managing a chronic condition can be more difficult during these unprecedented times. Your Horizon Blue Cross Blue Shield (BCBS) medical plan offers access to the Horizon Care Navigator/Primary Nurse Program to help support managing chronic conditions, such as diabetes, asthma and heart disease. They can also provide assistance with severe and complex care needs, including cancer, high risk pregnancies/NICU care, transplants and infusion therapy. The program’s trained case managers are fully licensed health care professionals and registered nurses.

Medical Coverage

Your medical plan is always available to support you and your covered family members with comprehensive coverage. If you need COVID-19 testing, that expense will be fully covered at no cost to you, as long as your physician provides a prescription. In addition, all of our medical plans include an out-of-pocket maximum to protect you from extensive costs if you need lengthy medical care.

The Food and Drug Administration (FDA) has granted Emergency Use Authorization for three COVID-19 vaccines. This continues to change as new vaccine developments are made, and as the vaccines become more widely available. DSM employees and their covered dependents enrolled in a Horizon Medical plan will pay no out of pocket costs for the COVID-19 vaccine or its administration.

Prescription Drug Coverage

Take advantage of your Express Scripts or Express Scripts’ specialty pharmacy Accredo home delivery program to receive a three-month supply of your maintenance medication without needing to leave your home.

Making Changes to Your Health Coverage

If you experience a qualifying life event — such as if your spouse loses his or her employer benefits, or if you have a baby or get married — you can make changes to your benefits coverage within 30 days of the event. Keep in mind that the changes must be related to your event. For example, if your spouse loses benefits coverage and you’re not currently enrolled in a DSM medical plan, you can newly enroll yourself and any eligible dependents; if you are already enrolled in a DSM medical plan, you can add your spouse to your current plan, but you cannot change which plan you are currently enrolled in.

Brighter Living Solutions at DSM – Employee Assistance Program (EAP)

The DSM Brighter Living Solutions at DSM, provided by Magellan Healthcare, is a free, confidential benefit to help you and your family members through times of stress, like the situation we are all in right now. Short-Term Disability

Family Leave

If you need to take a leave from work to care for a family member or to recover from your own illness, the Family and Medical Leave Act (FMLA) entitles you to take up to 12 weeks of unpaid, job-protected leave per year.
