• You have 90 days to use any remaining FSA balance. Contact Wageworks FSA at 1-888-557-3156.
• Port or Convert your Life and/or AD&D policy, if desired (31 days to complete); contact Prudential at 1-800-524-0542
• Review the Termination letter mailed to your home address on file for important information about your benefits. For questions, contact the DSM Benefits Center at 1-866-353-9740
