Health & Welfare Benefits

You have 30 days from your date of hire to enroll in benefits. If you are enrolled elsewhere and do not wish to enroll in DSM benefits, you are required to waive coverage within 30 days. If you do not complete this action before the deadline, you will be automatically enrolled in the DSM Consumer Directed Health Plan (CDHP) in employee-only coverage, and you will be subject to the Tobacco surcharge. You will not have another opportunity to enroll or change your benefit elections until Annual Enrollment (usually in the Fall), unless you experience a Life Event or meet special enrollment criteria.

As a new DSM employee, you will receive an email invitation to participate in our virtual New Hire Orientation. These onboarding sessions are held weekly on Tuesdays and you are invited to join the meeting which is most convenient for you, but encourage you to join as early as possible to ensure you complete enrollment within the 30-day deadline. In preparation for this meeting, we suggest you review this website in detail, as well as other onboarding materials.

You will also receive a New Hire letter with important information about your DSM benefits coverage, how to enroll and the timing of when you will receive additional materials including ID cards.

In addition to the New Hire letter, look for an enrollment kit from Fidelity Investments providing you information on our DSM 401(k) retirement benefits.

How to enroll

Within the first week of your hire, you will receive an email invitation to enroll from the DSM Benefits Center. After reviewing your benefit options, enroll or waive within 30 days of your date of hire. As a new user, you will need to register for a username and password to access the enrollment site. Step by step registration instructions can be found here. Registration code: mydsm-benefits.

You have 3 ways to enroll:

  • Web - DSM Benefits site - Enroll online at your convenience, any time of day or night.
  • Phone - 1-866-353-9740, Monday through Friday 9:00 a.m. to 7:00 p.m. ET.
  • App - Download the benefits app “ADP Mobile Solutions.” Follow the login process using the same login details as per the website. Note, you must have already registered for the website before you can use this app.

You can also email questions to DSMbenefitscenter@adp.com. Please allow 24-48 hours for a response, not including weekends. Note: Due to the privacy requirements of submitting questions relating to benefits, the email response will be sent via a secure email which requires you to create a username and password to open the email.

Complete All Your Post Enrollment Actions
  • If enrolling online or via the app, don’t forget to fully submit your elections. Your elections are not completed if you click ‘Finish Later’ or do not agree to the to the terms of enrollment. A green message and confirmation number will appear indicating you have finalized your enrollment. Save a copy of your confirmation statement as a record of your enrollment, to compare against the deductions in your pay check.

  • Be sure to provide the necessary dependent information to enroll eligible family members. If newly enrolling dependents for health care coverage, you will be required to provide your spouse’s Social Security number during the enrollment process. You will also be required to submit proof of your dependents’ eligibility after you enroll. The benefits enrollment site will prompt you to complete the dependent verification process online for each dependent and will include a list of the specific documentation required based on each dependent. For example, if you enroll a child, you will need to submit a birth or adoption certificate. If you enroll a spouse, you will need to submit a marriage certificate. (Domestic Partners require recent documentation showing a common address and intermingling of finances such as a utility bill and a bank statement which includes both individual’s names). Failure to respond will result in a retroactive termination of coverage without reimbursement of premiums paid.

  • At the time of hire, you have the opportunity to elect certain benefits such as Optional Life, Long Term Disability Buy Up, and Spousal Life Insurance without providing proof of good health. If you elect coverage now, you and your dependents can enroll for coverage up to a specified amount (“guaranteed issue”) without providing Evidence of Insurability (EOI). The enrollment system will confirm how much coverage you can elect before EOI applies. If you have elected Life and/or AD&D Insurance above the guaranteed issue amount, you must complete and submit requests for EOI to Prudential. You will receive this request within the enrollment site, immediately following submission of your elections. Should you prefer to fill out the form and return it to Prudential via fax, you will find the EOI form on the “Resources” page of this site. Follow the directions on the form.

  • Make sure you’ve provided beneficiary information for your life insurance and your DSM 401(k) Plan.

New Hires During and After Annual Enrollment

If you are hired during (or after) Annual Enrollment, you need to enroll for your new hire event for elections through December 31, and then immediately complete Annual Enrollment elections for the next plan year. When enrolling online, the Annual Enrollment window will open immediately following completion of your New Hire enrollment. It is important that you complete both enrollments even if your elections are the same, since you are required to confirm your Tobacco status and your enrolled spouse’s coverage status (if applicable). If both are not completed and you do not answer these questions, you will automatically be subject to the surcharges associated with each. (See Tobacco Surcharge and Spousal Surcharge sections below).

Tobacco Surcharge

If you and your spouse/DP enroll in a DSM medical plan, you will be required to attest to whether you have used tobacco products in the past 6 months. If you are a tobacco user, or do not attest to being tobacco-free during enrollment, you and your covered spouse/DP will each pay a $300 surcharge (added to your medical premiums annually).

Spousal Surcharge

The spousal surcharge is an additional premium that will apply if your spouse/DP has access to medical coverage through his/her employer and enrolls in a DSM medical plan. If you cover your spouse/DP under a DSM medical plan, you must certify that your spouse/DP does not have other employer coverage available. If you do not certify, or if your spouse/DP has access to their own employer’s medical plan you will pay a $1,200 surcharge per year - that is $100 per paycheck if you are paid monthly, $46.16 per paycheck if you are paid biweekly or $23.08 if you are paid weekly.

Rules for DSM Married Couples
  • Spousal surcharge does not apply.
  • Employee and spouse cannot cover each other for Life Insurance or AD&D but can participate in the same Medical Plan as Employee plus Spouse or Family coverage.
ID Cards

ID cards are generated for medical, prescription drug and dental coverage. There are no ID cards for vision coverage. If you enroll your dependents in medical and/or dental coverage, you will receive up to two ID cards in your name from each health plan provider. If you require additional cards, please contact the service provider directly. You can also print a copy or download your digital ID card via the respective provider’s smartphone app.

New Hires enrolling in DSM benefits are covered as of their date of hire. The process for your enrollment information to pass to the health care providers can take up to 2 weeks from the date you finalize your benefits enrollment (not your hire date). Please keep this in mind when contacting the providers and if any medical appointments are needed during this period. Your ID card may take another week to arrive by mail, following receipt of your enrollment is complete. If you need to make an appointment, print a copy of your ID card or download via the respective provider’s smartphone app. If the provider or pharmacy has not yet received your enrollment, they may require that you initially pay out of pocket for these expenses until your enrollment information is activated. You may then submit a claim form for these expenses to be reprocessed and reimbursed.

Lookout for Your Voluntary Benefits Email

If you elect any of the DSM voluntary benefit programs (such as the Legal Plan, Pet Insurance or ID Theft benefits), you will receive an introductory email from the provider inviting you to register on their website and complete next steps to initiate services. Please allow 1-2 weeks for receipt from the date your enrollment is submitted. If you do not receive an email, be sure to contact the provider directly. All DSM providers’ contact information can be found by navigating to the Contacts page of this website by using the drop-down menu.

After you have enrolled for DSM benefits coverage, you may find that you need to call a provider directly regarding a claim or coverage question. All of our service providers contact information is available on this website within the Contacts section.
